The Medical Clinic
Founded in 1995, Commonwealth Health Free Clinic provides basic medical diagnostics and treatment, labs and prescription medications to qualifying patients through the volunteer service of doctors, nurse practitioners, pharmacists, nurses, lab technicians and other volunteers. The Medical Clinic is funded through the financial support of Commonwealth Health Corporation and the generous support of local businesses and individual donors across the coverage area.
Basic Eligibility for Medical, Lab and Pharmacy Services
- The patient must reside in Allen, Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Monroe, Simpson or Warren counties.
- The patient (or patient’s legal spouse) must be currently and actively employed and must meet family financial criteria established by the Free Clinic.
- The patient must provide proof of employment at every visit. The Free Clinic will accept paycheck stubs as preferred documentation, but may accept other types of proof as evidence of employment, at the Clinic’s discretion.
- Patients with Medicaid, Medicare, Social Security disability benefits, unemployment benefits or other private pay insurance, including military medical benefits, are not eligible for services.
- Patients must be US citizens or documented residents of the United States.
- Patients must produce updated annual Federal Form 1040 or sign a 4506-T to be eligible for prescription assistance. Patients with annual family incomes above 185% of the federal poverty guideline are not eligible for services.
- Prescription and lab services and any referral services are provided as outcomes of actual patient visits to the clinic. The Free Clinic does not fill prescriptions written at outside doctor’s offices. Routine labs are also provided by order of the volunteer doctors who see patients in the Free Clinic setting.
